• The First Name and Age of the birthday child;
• The Date and Time you would like to have the show;
• The exact Location of the Show - for off-base residences, a Street Address including your apt./house number would be helpful.
• Will the show be in a Party Room, Community Room, or Private Residence?
• How many children (approximately) will be in attendance?
• What is the Range of Ages (what are the ages of the children in attendance)?
• How will you pay? (Cash, check, or Pay Pal)
• (OFF-BASE ONLY) Is Parking available nearby so I may unload my magic materials and apparatus?

• The show is one-hour long, which includes a 45 minute stage magic show followed by 15 minutes of balloon making;
• 45 minutes of UNINTERRUPTED set-up time is needed in order for me to prepare the magic tricks before the show;
• FOR SHOWS IN YOKOSUKA/IKEGO PARTY ROOMS: The Housing Office normally won't let you make a reservation more than two months in advance. Please reserve one table for my use if that is possible;

• Your child will feel special in front of friends and family since I feature your child in the show on stage in many tricks. This is an excellent opportunity for you to take photographs or videos to capture this special moment;
• The Birthday Child will receive a Complimentary Special Magic Gift from me;
• For the comfort and viewing of the audience, it is best to arrange to have the children seated in chairs, not sitting on the floor;
• It is best if the children are not eating, carrying drinks, or holding balloons/toys during the show (this is to avoid distracting the audience and increases the likelihood of that child being called to join the stage show);
• Please advise me if you are planning to rent a Blow-Up/Bouncy Castle. They take up almost half of the party room and can get quite noisy during the show if it is running.
• Ideally an area of 3 meters by 2 meters ( 9 ft by 6 ft) needs to be cleared for the Magic Stage Area. This does not include the area for the children to be seated comfortably;
• It takes about 30 minutes to pack my belongings after the show. I normally do this while my wife/assistant makes the balloons for the children;
• Ideally I need to perform with my back to a wall - not to a door or entrance way. It's a magician thing!
• A slightly larger fee for long-distance travel (Yokohama, Tokyo, for instance) is charged since I am travelling by car on toll roads from Yokosuka;

• METHOD OF PAYMENT: You may pay me after the show is over - cash is preferred. I do accept checks written on NFCU or Community Bank accounts. You may also pay me via Pay Pal at mikeapplegate@hotmail.com. (If you decide to pay by Pay Pal please contact me to make arrangements to pay in advance .)
